Commercial spots for GoHealth and their partners in specific markets: Legacy Health (Oregon), Hartford Health (Connecticut), Northwell Health (NY/NJ Tristate), Dignity Health (Northern CA San Francisco/Oakland/San Jose).
Initial session (one day 10-hour shoot + wardrobe fitting + behind the scenes footage for “making of” video) plus usage pays $1,250 (+20% rep fee). Additional payments for travel days, hold days, overtime after 10 hours, possible 2nd shoot day (not all roles – some will be one day only), and possible print shoot (not all roles) – also possible additional options – read further. Buyout included is 2 years for internet usage and regional broadcast for four regions: NY/NJ (Tristate), OR (Portland), CT (Hartford/New Haven), and Northern CA (San Francisco/Oakland/San Jose). All actors must agree to these buyout terms. Please check your conflicts – we are not holding for any conflicts, however please do not submit if you have another conflict with another healthcare service from a previous job.
Overtime (double-time) for 11 and 12 hours to be paid in 30 minute increments based on a rate of $550 for 10/hours – calculated at $55/hour (so double-time would be paid at $110/hour). $55.00 for every 30 minutes of work after 10 hours. 30-minute lunch break deducted from work day hours.
Additional work day session fees (2nd shoot day) paid at $550 (+20% fee) for 10 hours, with the same OT provisions listed above.
Travel days and hold days paid at $275 + 20% fee (50% of $550 rate). Minimum of 2 travel days included per role, unless actors reside in Portland area.
Any additional regional markets (in addition to the four listed above) to be paid at $550 (+20% fee) per market (for same regional broadcast + internet use for 2 years).
ADDITIONAL BROADCAST OPTIONS:
- Option #1 – $750 + 20% to extend usage for 3 additional years (for a total of 5 years) for internet use and broadcast for the four regional markets listed above: NY/NJ (Tristate), OR (Portland), CT (Hartford/New Haven), and Northern CA (San Francisco/Oakland/San Jose).
- Option #2 – $2,250 + 20% to upgrade usage to national broadcast and internet use for three years.
- Option #3 – $3,250 + 20% to upgrade usage to national broadcast and internet use for five years.
For print session fee ONLY, usage to be paid by client after usage is determined. (TBD – not all roles will be included in the print shoot.) Print day rate of $550 + 20% for 10/hours – calculated at $55/hour (so double-time would be paid at $110/hour). $55.00 for every 30 minutes of work after 10 hours. 30-minute lunch break deducted from work day hours.

- Print Option #1 – $1500 + 20% for national print buy of 3 years, no OOH, no POS.
- Print Option #2 – $500 + 20% for national POS buy of 3 years.

Thin and feminine, but physically capable. A girl who is naturally pretty, but a no-nonsense, do-it-herselfer. Someone who was probably a tomboy as a kid who grew into a beautiful woman. Someone like Tristin Mays or Nathalie Emmanuel.
[EAR INFECTION: MOTHER] Female. Late-30’s. Caucasian. Brunette. Attractive. A woman with a career and two kids. Someone who can do the whole executive thing, but at the end of the day is a bad ass woman who loves her children and can take action. Maybe someone like Emilia Clark or Ellen Page.
[EAR INFECTION: BOY] Male. 5-7 years old (should be small, so they read younger). Caucasian. Brunette. Sweet. Cute. Adorable (so we feel bad when he’s in pain). Needs to look like our mom is his mom. Would be good to cast him in his PJ’s.
[SUTURES: GUY} Male. Late-30’s. Caucasian. Brunette. Works at an office, but has a love for cars and restoring them. He isn’t the stereo-typical car maintenance blue collar guy, but does have a bit of a rugged streak in him. Handsome. Fairly thin and looks good dressed for business, but loves nothing better than wearing a t-shirt and jeans. That’s a Sam Rockwell or a Ewan McGregor type.
[MEMBERSHIP: WIFE] Female. Mid-30’s. Caucasian. Brunette. Someone who is practical and a bit on the conservative side. Maybe an Allison Frasca or an Ali Gordon type.
[MEMBERSHIP: HUSBAND] Male. Mid-40’s. Caucasian. Prematurely balding, but has a nice style about him. Probably a lawyer or an accountant. Totally dedicated to his wife and career. A slight introvert, but fun loving. Think Bryan Cranston.
